29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws: a. On or about February 28, 2018, located in the Maintenance Shop, employees operated a Rigid pipe threader that was supplied with electrical power from an extension cord that was plugged into a portable electrical outlet box that did not have strain relief provided for its flexible cord, thereby exposing the employees to an electrical shock hazard of 120 VAC. b. On or about April 10, 2018, located in the Mandrel Hose Geese Finishing Room, employees operated a Dayton pedestal grinder that had a flexible cord that did not have strain relief provided at its plug, thereby exposing employees to an electrical shock of 120 VAC. c. On or about April 19, 2018, located in the Vulcanizing Department at Smaco splicing press #37, there was a flexible cord that provided power to the heater box that did not have strain relief provided at its cord plug, thereby exposing employees to an electrical shock hazard of 120 VAC.
