29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing and labeling. On or about October 08, 2014, and at times prior thereto, metal boxes with knockouts, designed to be mounted, were used as part of shop-built portable extension cords found throughout the facility, exposing employees to the hazards of electrical shock.
