29 CFR 1910.305(b)(2)(i): The employer did not ensure each outlet box in completed installations had a cover, faceplate or fixture canopy: ALON USA, LP, 200 Refinery Road, Big Spring, TX. The employer does not ensure each outlet box had a cover. This violation was observed on or about April 15, 2009 in the following instance: Three outlet receptacles located in the DHT Motor Control Center room were missing their covers. A Bell Electrical Products multiple outlet power strip 125 VAC - 15 A - 60 Hz No. 851 was plugged into one of the uncovered outlets located under a work bench. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that each outlet box in completed installations has a cover.
