29 CFR 1910.254(b)(4)(iv): Terminals for welding leads were not protected from accidental contact by personnel or metal objects: On or about March 29, 2018, employees are required to utilize a step ladder to gain access to the Miller welder inside the fabrication shop. The employees climb the ladder to operate the power switch daily which places them in direct proximity to the un-guarded terminals, exposing employees to an electrical shock hazard.
