29 CFR 1910.119(l)(4): A change covered by 29 CFR 1910.119(l) resulted in a change in the process safety information required by 29 CFR 1910.119(d) and process information was not updated: a- Hess Port Reading Refinery, FCCU - The P&ID for the Steam Condensate & BFW Distributor, Job No. 6411-05, Drawing No. 64-D4 was not updated to reflect the addition of snuffing steam to P-502A/B, P-508A/B, P-509A/B, P-516A/B and P- 517A/B as part of project no. 1377. Condition noted on or about 02/10/09. b- Hess Port Reading Refinery, FCCU - The P&ID for the New & Existing Air Blowers & Heaters, Job No. 6411-05, Drawing No. 62-D3 was not updated to reflect the new RV set pressures for the C-502 Axial Air Blower. RV-C-502-1, RV-C-502- 2, RV-C-502-3 and RV-C-502-4 were recorded on the P&ID at 46 psi, 46.7 psi, 47.4 psi and 48.3 psi when the correct settings should have been 44 psi, 44.7 psi, 45.4 psi and 46.2 psi, respectively. Condition noted on or about 03/13/09. c- Hess Port Reading Refinery, FCCU Flue Gas Cooler - Sections of 12 inch piping connecting to the atmospheric vents had been cut and removed from the D-514 High Pressure Disengaging Drum with the remaining out of service piping still in place on the drum. Such changes were not noted on P&ID 64-D1. Condition noted on or about 12/16/08.
