Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to slips, trips, and falls due to insufficient illumination provided in the work area: a) Rolling Division 1) Slab Yard #2 and #3 - Second and third shift employees were exposed to potential tripping hazards as the quality and quantity of illumination was poor or almost nonexistent (e.g. slab yard #2 had approximately 52 burnt out light bulbs and the south end of this yard had no lighting fixture installed; slab yard #3 had even less illumination than yard #2), observed September 19, 1989. b) Steel Producing Division 1) Open Hearth Department - Employees were exposed to the hazard of slips, trips, and falls due to insufficient illumination provided at Open Hearth employee walkway to parking lot. Illumination readings indicated from 0.1 to 4.0 footcandles of lighting, observed September 13, 1989. 2) Open Hearth Department - Employees were exposed to the hazard of slips, trips, and falls due to insufficient illumination provided at steps down from Open Hearth #8 and #9 furnaces. Illumination readings indicated 0.1 footcandles of lighting, observed September 13, 1989. 3) Open Hearth Department - Employees were exposed to the hazard of slips, and falls due to insufficient illumination provided behind #9 Open Hearth Furnace. Illumination reading indicated 0.5 footcandles of lighting, observed September 13, 1989. 4) Open Hearth Department - Employees were exposed to the hazard of slips, trips, and falls due to insufficient illumination provided at #8 Open Hearth Furnace facing east. Illumination reading indicated from 6.0 to 2.0 footcandles of lighting, observed September 13, 1989. 5) Open Hearth Department - Employees were exposed to the hazard of slips, trips, and falls due to insufficient illumination provided at #3 turnout at #9 Open Hearth Furnace. Illumination reading indicated 8.0 footcandles of lighting, observed September 13, 1989. 6) Open Hearth Department - Employees were exposed to the hazard of slips, trips, and falls due to insufficient illumination provided south of material bin #22 bin. Illumination reading indicated from 0.2 to 14.0 footcandles of lighting, observed September 13, 1989. 7) Open Hearth Department - Employees were exposed to the hazard of of slips, trips, and falls due to insufficient illumination provided in the work area. Illumination readings indicated from 5.0 to 8.0 footcandles of lighting at the south end walkway of the Stockhouse, observed September 12, 1989. c) Parking Lot 1) Outside Parking Lot Area Adjacent to Outside Plant Entrance Gate - Employees were exposed to the hazard of slips, trips and falls due to insufficient illumination provided in the work area. Illumination readings indicated 0.1 to 2.5 footcandles of lighting observed September 13, 1989. Among other methods, one feasible and acceptable abatement method to correct this hazard is to provide adequate illumination in the area, as recommended by the Illumination Engineering Society, A11.1
