Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer does not furnish employment and a place of employment which are free from recognized hazards that are causing or likely to cause death or serious physical harm to employees in that employees are exposed to struck-by hazards: a)The employer does not ensure that employees utilize a floor mounted jib crane to hoist loads that are within the rated load limits of the hoist. This occurred on October 29, 2010, when jib crane JC11 (not marked with the manufacturer or serial #), with a rated load capacity of 500 lbs, was used with a Dayton hoist, model, HHXG12, serial # 4GU71 with a rated capacity of 500 lbs, to hoist a frac adapter 909879RNT, weighing 968 pounds. This lift overloaded the hoist, exposing employees to struck by hazards. b)The employer does not ensure that hoists are inspected frequently and periodically. This occurred on October 21, 2010, resulting in hoists being used with hook safety latches missing, and hoist pendant stations that were not legibly marked. These conditions exposed employees to struck-by hazards. ABATEMENT NOTES: Among other methods, one feasible method of abatement to reduce this hazard is to ensure loads do not exceed the rated load of hoisting equipment as outlined in the American Society of Mechanical Engineers, ASME B30.16-1998 Section 16-3.2.1 which indicates that a hoist shall not be loaded in excess of its rated load except for test purposes as provided by paragraph 16-2.2.2 or for planned engineered lifts as provided in paragraph 16.3.2.2, and to inspect cranes and hoists frequently and periodically, as outlined in the American Society of Mechanical Engineers, ASME B30.16-1998 Section 16-2.1.b Inspection Procedures for hoist in regular service. 16-2.1.2 Frequent inspections shall be performed at intervals defined in 16-2.1.1 (daily, weekly and monthly). Section 16-2.1.3 states that periodic inspection shall be performed at intervals defined in paragraph 16-2.1.1(b)(2) - yearly for a crane in normal service. c)The employer does not ensure that floor mounted jib cranes used to hoist loads are used within the rated load capacity of the crane. This occurred on October 21, 2010, when jib crane JC11, with a rated load capacity of 500 lbs, was used with a Coffing hoist, with a rated capacity of 1,000 pounds to hoist a dual actuator, part number 903515, weighing 852 pounds. This lift overloaded the crane, exposing employees to struck by hazards. d)The employer does not ensure that cranes are inspected frequently and periodically. This occurred on October 21, 2010, resulting in cranes being used that were not marked with rated capacity. This condition exposed employees to struck-by hazards. ABATEMENT NOTES: Among other methods, one feasible method of abatement to reduce this hazard is to ensure loads do not exceed the rated load of cranes as outlined in the American Society of Mechanical Engineers, ANSI/ASME B30.11 Monorails and Underhungs cranes, 11.1.1 which indicates that a hoist shall not be loaded in excess of its rated load except for test purposes, and 11.1.2 which contains inspection requirements for cranes in regular use. The employer is responsible for selecting and carrying out an appropriate and effective method of abatement. Pursuant to 29 CFR 1903.19, within (10) Calendar days of the abatement date of this citation, the employer must submit documentation describing the actions taken to ensure that crane and hoist capacities are not exceeded, and that crane and hoist inspections are conducted frequently and periodically.
