29 CFR 1910.119(l)(1): The employer did not establish and implement written procedures to manage changes to process chemicals, technology, equipment, and procedures; and, changes to facilities that affect a covered process: c- Hess Port Reading Refinery, FCCU - An MOC was not written to allow T-501 to operate with cementitious fireproofing missing from pipe support tie-ins found on the tower's skirt. Condition noted on or about 02/27/09. d- Hess Port Reading Refinery, Alky - An MOC was not written to allow T-703 to operate with cementitious fireproofing removed from the upper circumference of the skirt. Condition noted on or about 02/27/09. e- Hess Port Reading Refinery, Alky - An MOC was not written for the use of a temporary pipe support for a 6 inch overhead line on E-711C. Conditions noted on or about 02/27/09. f- Hess Port Reading Refinery, FCCU - An MOC was not written for a temporary pipe support on an insulated line on top of T-503 Naphtha Splitter. Condition noted on or about 02/27/09. g- Hess Port Reading Refinery, SRU - An MOC was never written when a check valve was installed on the MEA acid gas bypass line. On 02/28/08 this check valve failed allowing hydrogen sulfide to back flow through the line and overcome an operator. Condition noted on or about 02/06/09. h- Hess Port Reading Refinery, Gas Con - An MOC was not written for the "Arkansas Cooler" used on the E-635A/B/C Debutanizer Bottoms Cooler on or about 01/05/09.
