29 CFR 1910.261(k)(1): Emergency stop buttons and pull cords on the paper machines were not located in such a matter that was easily accessible for any person working on the machine to quickly disconnect the machine from the source of power in case of emergency: Paper Machine #9- Employees were not protected from severe laceration, amputation and crushing hazards where emergency stop buttons and pull cords were not located in such a manner that was easily accessible for employees working on the paper machine on the main working level and in the basement. On or about November 21, 2019, employees in the basement were reaching into the paper machine, using a hose to spray water on the crepe set roll, to make a guide adjustment where emergency stop buttons and pull cords were not located.
