29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings shall be effectively closed: a. On or about February 28, 2018, located in the Finishing Room above and to the right of the miter wet saw, there was an electrical junction box that had a knockout that was not effectively closed, thereby exposing employees to an electrical shock hazard of 120 VAC. b. On or about February 28, 2018, located in the finishing room near the drill press, there was a 440 VAC electrical disconnect box that had an unused opening on its bottom side, thereby exposing the employees to an electrical shock and/or arc flash hazard. c. On or about March 7, 2018, located in the Maintenance Shop, employees operated a Rigid pipe threader that was supplied with electrical power from an extension cord that was plugged into a portable electrical outlet box. The portable electrical outlet box had one unused opening on the back and one on the bottom of the box, thereby exposing the employees to an electrical shock hazard of 120 VAC. d. On or about March 8, 2018, located in the Mandrel Hose Geese Finishing Room, there was a 4-11/16 metal electrical receptacle outlet box that had an unused opening where a knockout had been removed that was to the right of wet saw #8, thereby exposing the employees to an electrical shock hazard of 120 VAC. e. On or about April 19, 2018, located in the lab, there 220 VAC electrical panel box that had a missing breaker that was not filled with another breaker or blank, thereby exposing the employees to an electrical shock and/or arc flash hazard.
