29 CFR 1926.502(j)(1): Toeboards were not erected along the edge of the overhead walking/working surface for a distance sufficient to protect employees below: a- 6th Floor Elevator Shaft - 642 Bedford Avenue, Brooklyn NY: On or about 1/14/16 site workers were exposed to struck by injuries when the side of the shaft facing the rear of the building and the opening of the elevator shaft were not supplied with toe boards. b- 5th Floor Elevator Shaft - 642 Bedford Avenue, Brooklyn NY: On or about 1/14/16 site workers were exposed to struck by injuries when the side of the shaft facing the street and the other side facing the rear of the building were not supplied with toe boards. c- Basement - 642 Bedford Avenue, Brooklyn NY: On or about 1/14/16 site workers were exposed to struck by injuries when the side of the shaft facing the street and the opening of the elevator shaft were not supplied with toe boards. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
