29 CFR 1910.146(k)(3): Whenever an authorized entrant entered a permit space, retrieval systems or methods were not used to facilitate non-entry rescue: a) On or about November 29, 2012, Phoenix Industrial Cleaning, Inc. did not ensure, during the permit space entry, that retrieval systems or methods were used to facilitate a non-entry rescue when an employee entered a 6,000 gallon 16 foot vertical chemical storage tank at Sunnyside Corporation in Wheeling, Illinois. This tank was last used by Sunnyside Corporation to store methylene chloride.
