29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment paragraph (d)) before further use or in a safe and timely manner: a- Hess Port Reading Refinery, FCCU - Cementitious fireproofing was removed and not replaced on the T-501 Main Column when steel pipe supports were connected to the tower's skirt. Condition noted on or about 02/27/09. b- Hess Port Reading Refinery, FCCU - Motor operated valves (MOVs) for the P- 518A/B Main Column Bottoms Pump and associated electrical conduit was not fireproofed. Condition noted on or about 12/15/08. c- Hess Port Reading Refinery, FCCU - Cementitious fireproofing had fallen from a section of structural steel above E-510 LCO Product/Raw Oil Exchanger leaving a section of the steel unprotected. Condition noted on or about 12/16/08. d- Hess Port Reading Refinery, Alky - Cementitious fireproofing had been removed from the upper skirt of the T-703 Debutanizer spanning the tower's entire circumference. Condition noted on or about 02/27/09. e- Hess Port Reading Refinery, Dimersol - The RD for P-2202A 2nd Reactor Pump Around had burst as indicated by a reading of 350 psi on the tell tale pressure gauge. Condition noted on or about 03/05/09. f- Hess Port Reading Refinery, Dimersol - The tell tale pressure gauge for S-2202B Caustic Rag Filter was reading 14 psi indicating a potential leak through the RD. Condition noted on or about 03/05/09. h- Hess Port Reading Refinery, Alky - The snuffing steam line for P-701B Effluent Recycle Pump was not installed properly. The pipe outlet was not pointed at the pump seal making it ineffective in extinguishing a potential seal fire. Condition noted on or about 02/27/09. i- Hess Port Reading Refinery, FCCU - The P-506B LCO Circulating Pump was operated with a bent and damaged grounding wire. Condition noted on or about 12/15/08. j- Hess Port Reading Refinery, Gas Con - The D-609 Lean Gas Knock Out Drum RV Bypass High Point Vent was missing a gasket and the flange was improperly secured with bolts missing their nuts. Condition noted on or about 12/18/08. k- Hess Port Reading Refinery, Gas Con - The pressure gauge for the P-602B Stripper Charge Pump was inoperative with a reading 600 psi. Condition noted on or about 12/18/08.
