29 CFR 1910.119(o)(4): The employer did not promptly determine and document an appropriate response to each of the findings of the compliance audit, and document that deficiencies have been corrected: (a)Suncor Energy (U.S.A.), Inc., 5801 Brighton Blvd., Commerce City, Colorado: On or about October 21, 2008, only 13 of 39 audit findings from the previous 2007 and 2005 audits had been corrected and documented. Employees have increased exposure to the hazards of hydrogen sulfide, fire, and explosion due to the employer's failure to correct identified deficiencies in the PSM program.
