29 CFR 1910.334(a)(3)(ii): Attachment plugs and receptacles were connected or altered in a manner which prevented proper continuity of the electric equipment grounding conductor at the point where plugs were attached to receptacles: Spray Room, Upstairs: On, or about, December 1, 2014, a ceiling mounted Edison base light fixture had been converted to a receptacle using a screw-in adaptor. In order to energize a three way splitter that in turn energized extension cords and light fixtures, a "cheater plug" was plugged in between the screw-in adaptor and the three way splitter.
