29 CFR 1926.405(g)(2)(v): Flexible cords and cables were not protected by bushings or fittings where passing through holes in covers, outlet boxes, or similar enclosures: a- 6th Floor - 642 Bedford Avenue, Brooklyn NY: On or about 1/14/16 site workers were using a flexible cord fed through a hole in an outlet box without being protected. b- 1rst Floor - 642 Bedford Avenue, Brooklyn NY: On or about 1/14/16 site workers were using two flexible cords fed into a circuit breaker box without being protected. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
