29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables used for temporary wiring were not protected from damage: a- 6th Floor - 642 Bedford Avenue, Brooklyn NY: On or about 1/14/16 site workers were used flexible cords fed through and around the facility's elevator shaft to supply power to the 6th floor junction box. This cord was crushed by debris, tripped over by worksite and company employees, etc. and was not protected from being damaged. b- 5th Floor - 642 Bedford Avenue, Brooklyn NY: On or about 1/14/16 site workers were using flexible cords were crushed by debris, tripped over by worksite and company employees, etc. and was not protected from being damaged. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
