29 CFR 1910.119(j)(4)(ii): Inspections and testing procedures performed on process equipment to maintain its mechanical integrity did not follow recognized and generally accepted good engineering practices (RAGAGEP): a- Hess Port Reading Refinery, FCCU, Gas Con, Alky - The employer did not comply with API 510 Section 6.6 requirements when it failed to reduce the inspection frequency of pressure relieving devices which were heavily fouled or stuck in the last inspection or test. Such valves included but were not limited to RV-D-603-1, RV-E- 603-RT, RV-E-610-R, RV-E-622-T, RV-E-627, RV-S-603-2, RV-T-620-1, RV-D- 710, RV-E-708-T, RV-P-712A, RV-P-722, RV-E-501-BR-2S, RV-E-502-BR-T, RV- E-504-R-S, RV-P-501A/B, RV-S-512 and RV-SC-559-1. Conditions noted on or about 03/03/09 and 03/09/09. b- Hess Port Reading Refinery, FCCU, Gas Con, Alky - The employer did not comply with API 510 Section 6.6 requirements when it allowed inspection intervals on equipment RVs in fouling service to exceed 5 years. Such valves included but were not limited to RV-S-603C, RV-D-612, RV-E-608-R, RV-T-622, RV-D-705-1, RV-P-712B, RV-T-702A, RV-D-507-R-1, RV-E-502-BR-T and RV-E-504-R-S. Conditions noted on or about 03/03/09 and 03/09/09. c- Hess Port Reading Refinery, FCCU, Gas Con, Alky - The employer did not comply with API 574 Section 6.3.3.2 requirements when it failed to ensure that insulation plugs were replaced and sealed after taking piping thickness measurements on insulated piping. Insulation plugs were missing from process piping found in but not limited to the following drawings: 700-07A, 700-7B, 700-08, 700-30A, 700-52, 700-54, 700-66, 700-73, 700-76, 700-96, 600-24, 600-45, 600-112, 600-132, 600- 133, 500-5, 500-08A, 500-9, 500-13, 500-13, 500-18, 500-27, 500-39, 500-45, 500- 47 and 500-56. Condition noted on or about 12/16/08 and 02/27/09.
