29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the following means: 1. By location in a vault, or similar enclosure that is accessible only to qualified persons. 2. By suitable permanent, substantial partitions or screens so arranged so that only qualified persons will have access to the space within reach of the live parts. 3. By placement on a suitable balcony, gallery, or platform so elevated and otherwise located as to prevent access by unqualified persons. 4. By elevation of eight feet or more above the floor or other working surface. a. On or about March 21, 2018, located in the Vulcanizing Department, employees operated 12 pneumatic Thomas Bus mold presses that had their wiring spliced with wire nuts on the back of each press, thereby exposing employees to an electrical shock hazard form the exposed live parts of 240 VAC. b. On or about February 28, 2018, located at the Maintenance shop there was a splice in the flexible cords at an exhaust fan that was held together by wire nuts, thereby exposing employees to an electrical shock hazard of 120 VAC. c. On or about April 19, 2018, located in the Vulcanizing department, employees operated 18 SMACO splice presses that had its wiring spliced with wire nuts on the back of each press, thereby exposing the employees to an electrical shock hazard from the exposed live parts of 240 VAC.
