29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph. a- 6th Floor - 642 Bedford Avenue, Brooklyn NY: On or about 1/14/16 worksite employees were exposed to electrocution hazards when the facility temporary lighting strings contained open live sockets and were strung less than 8 feet above the floor/working surface. b- 6th Floor - 642 Bedford Avenue, Brooklyn NY: On or about 1/14/16 site workers were exposed to electrocution hazards when two receptacles were hanging by their wires outside of an electrical box. c- 5th Floor - 642 Bedford Avenue, Brooklyn NY: On or about 1/14/16 site workers were exposed to electrocution hazards when the facility temporary lighting strings; contained lighting strings with unprotected light bulbs, contained open live sockets, were strung less than 8 feet above the floor/working surface, were wrapped around wire rope strands for support, etc. d - 4rth Floor - 642 Bedford Avenue, Brooklyn NY: On or about 1/14/16 site workers were exposed to electrocution hazards when the facility temporary lighting strings; contained lighting strings with unprotected light bulbs, contained open live sockets, were strung less than 8 feet above the floor/working surface, were wrapped around wire rope strands for support, etc. e - 3rd Floor - 642 Bedford Avenue, Brooklyn NY: On or about 1/14/16 site workers were exposed to electrocution hazards when the facility temporary lighting strings; contained lighting strings with unprotected light bulbs, contained open live sockets, were strung less than 8 feet above the floor/working surface, etc. f - 2nd Floor - 642 Bedford Avenue, Brooklyn NY: On or about 1/14/16 site workers were exposed to electrocution hazards when the facility temporary lighting string contained unprotected light bulbs. g - 1rst Floor - 642 Bedford Avenue, Brooklyn NY: On or about 1/14/16 site workers were exposed to electrocution hazards when the facility temporary lighting strings contained lighting strings with unprotected light bulbs. h - 1rst Floor - 642 Bedford Avenue, Brooklyn NY: On or about 1/14/16 site workers were exposed to electrocution hazards when two separate, live circuit breaker panels were missing their covers. i - Basement - 642 Bedford Avenue, Brooklyn NY: On or about 1/14/16 site workers were exposed to electrocution hazards when the facility temporary lighting strings; contained lighting strings with unprotected light bulbs. j - Basement - 642 Bedford Avenue, Brooklyn NY: On or about 1/14/16 site workers were exposed to electrocution hazards when the live circuit breaker panel and a junction box were missing their covers. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
