29 CFR 1910.119(j)(4)(i): Inspections and tests were not performed on process equipment to maintain its mechanical integrity: a- Hess Port Reading Refinery, Dimersol - Rupture disks (RD) and associated tell tale pressure gauges were not inspected on process equipment to include but not limited to the S-2204A Stabilizer Feed Filter, D-2201 Caustic Wash Drum and D-2213 Ammonia Mix Drum. Tell tales/pressure gauges for RDs on the D-2202 Water Wash Drum and D-2203 Stabilizer Surge Drum were unreadable as they were etched and clouded over. Conditions noted on or about 03/05/09 and 03/31/09. b- Hess Port Reading Refinery, Gas Con - Readings were not taken at specified active thickness measurement locations (TMLs) for the T-622 Deethanizer. Condition noted on or about 02/20/09.
