29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service: a) 9000 River Rd, Delair, NJ, 08110- Waste Water Treatment: The flexible cord used to power the portable heating units contained a damaged plug casing and cord. It was not removed from service, on or about 3/15/2017. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19
