Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: On, or about, December 1, 2014, in the following locations, electrical boxes were not provided with proper covers A. Side Room, Front of Shop: A ceiling mounted junction box that was part of the wiring system for the building was not provided with a cover. B. Side Room, Front of Shop: A junction box that was mounted on the wall above a workbench was not provided with a cover. C. Side Room, Front of Shop: A receptacle that was mounted on an overhead storage rack was not provided with a cover. D. Storage Room: A ceiling mounted receptacle was not provided with a cover. E. Upstairs Prep Room: Two wall mounted receptacles were not provided with covers.
