29 CFR 1910.119(m)(3): An incident investigation team shall be established and consist of at least one person knowledgeable in the process involved, including a contract employee if the incident involved work of the contractor, and other persons with appropriate knowledge and experience to thoroughly investigate and analyze the incident: a. As of November 20, 2015, the incident investigation report for the incident, which occurred on or about December 2, 2014, and involved a vapor line from the beer column to the rectifier column that was not locked in the closed position and allowed ethanol vapor to enter the beer column during entry, was investigated by only one person in lieu of establishing an incident investigation team.
