29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: (a) Facility, Building 1, Machine Shop: The tongue guards were not properly adjusted on the Baldor pedestal grinder, W10-91. The left opening measured 1/2 of an inch away from the abrasive wheel and the opening on the right also measured 1/2 of an inch away from the abrasive wheel, on or about 5/13/2015. (b) 9000 River Rd, Delair, NJ- Mold Maintenance: The tongue guards were not properly adjusted on the eight inch Dayton pedestal grinder, on or about 4/16/15 NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.
