29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: a) On or about May 12, 2016, in Building 155 at Column B:8, electrical conveyor box had two unused openings, that were not effectively closed exposing employees to electrical shock and fire hazards. b) On or about May 12, 2016, in Building 155 at Column B:8, lower box, pipe conveyor box had two unused openings that were not effectively closed exposing employees to electrical shock and fire hazards. c) On or about May 12, 2016, in Building 155, the Vane Machine, had two receptacle boxes located below the working plane that had two unused openings. The two unused openings were not effectively closed exposing employees to electrical shock and fire hazards. d) On or about May 12, 2016, in Building 155 at Column H:9, Plate Roller, white electrical box had one unused opening that was not effectively closed exposing employees to electrical shock and fire hazards. e) On or about May 12, 2016, in Building 155 at Column H:10, white box had two unused openings that were not effectively closed exposing employees to electrical shock and fire hazards.
