29 CFR 1910.147(d): The established procedure for the application of energy cotnrol (the energy control procedure) was not done in sequence and it did not contain the elements as required by 29 CFR 1910.147(D)(1) through (d)(6): a- Bayway Chemical Plant, Lofi Unit - On 01/25/10 a mechanic was sprayed with heated product from the 7R20 Lofi Reactor while removing the 7E20 P/A Heater safety valve (7E20SV). Mechanics and the process operator failed to properly isolate the 7E20SV in that the 7P20 Recyle P/O Pump for 7R20 was operating and in line with the 7E20/7E20SV. In addition, a 3/4 inch block valve on the inlet to 7E20SV was not indicated on the P&ID and was not locked out.ed
