29 CFR 1910.213(r)(4): Suitable guard(s) were not provided to reduce to a minimum the hazard due to the point of operation of panel saw, mortiser, cutoff saw, mortiser, boring machines: a) Rough Mill, Front Room, on or about 4/15/15: Point of operation and hold down on a Cain panel saw were not guarded. b) Wood Shop, Turning Cell, on or about 4/15/15: Cutting bit on a Maka mortiser machine was not guarded. c) Wood Shop, on or about 4/15/15: Two Medalist Bell double end cutoff saw and boring machines did not have the point of operation adequately guarded and the hold downs were not guarded. d) Wood Shop, on or about 4/15/15: Point of operation at the boring bits/router and hold downs on an anchor/screw machine, Operation #28, were not guarded. e) Wood Shop, on or about 4/15/15: Point of operation at the boring bits and hold downs for the horizontal boring machines #3-3 and 3-19 were not guarded. f) Wood Shop, Corner Block, on or about 4/15/15: Point of operation on a corner block chop saw was not guarded. g) UFB Bldg., second floor, on or about 4/16/15: Point of operation at the boring bits and hold downs on an American horizontal boring machine were not guarded.
