29 CFR 1910.219(c)(4)(i): Projecting shaft ends did not present a smooth edge and end and projected more than one-half the diameter of the shaft without being guarded by nonrotating caps or safety sleeves. On or about October 08, 2014, and at times prior thereto, the employer did not ensure that projecting shaft ends on three saws in the Saw Department and one in the AmStock Area were guarded with a non-rotating cap or safety sleeve, exposing employees to the hazard of being caught in the rotating shaft ends.
