29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. Paper Machine #9, main working level- Employees were not protected from severe laceration, amputation, and crushing injuries where in running nip points and power driven rolls including, but not limited to, the main press and bottom felt loop roll were not guarded on the tending and drive side of the paper machine. b. Paper Machine #9, main working level- Employee were not protected from severe laceration, amputation, and crushing injuries where rotating parts and the in running nip point for Yankee dryer on the tending and drive side of the paper machine was not guarded. c. Paper Machine #9, basement- Employees were not protected from severe laceration, amputation, and crushing injuries where rotating parts and in running nip points for exposed rolls were not guarded on the tending and drive side of the paper machine. d. Maintenance shop- Employees were not protected from flying debris, severe laceration, and amputation injuries where the rotating chuck, shaft, and point of operation on the Universal boring machine were not guarded. e. Maintenance Shop- Employees were not protected from flying debris, severe laceration, and amputation injuries where the rotating chuck and point of operation on the Bridgeport vertical milling machine were not guarded. f. Maintenance Shop- Employees were not protected from flying debris, severe laceration, and amputation injuries where the rotating chuck, lead screw, and point of operation on the Republic Lathe, Leblond 30 foot lathe, Leblond Makino 14E-575 lathe, and Leblond Makino 14E-477 lathe were not guarded. g. Maintenance Shop- Employees were not protected from flying debris, severe laceration, and amputation injuries where the rotating chuck and point of operation on the Wilton Manufacturing drill press and the large drill press were not guarded.
