29 CFR 1910.147(c)(5)(ii) (D): The employer did not ensure that lockout / tagout devices indicate the identity of the employee applying the device(s): ALON USA, LP, 200 Refinery Road, Big Spring, TX. The employer does not ensure lockout / tagout devices identify the employee applying the device. This violation was observed on or about April 15, 2009 in the following instances: a. Tagout devices being used in the DHT Motor Control Room isolating the 80-WR-2 480V Welding Receptacle switch do not have the name of the operator which applied the tagout. b. Tagout devices being used in the #1 Sulfur Recovery Unit isolating two valves for the Quench Filter 68-FL002 do not have the name of the operator which applied the tagout. c. Tagout devices being used in the Sour Water Stripper unit isolating valve V-16T00 do not have the name of the operator which applied the tagout. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure lockout / tagout devices identify the employee applying the device(s).
