29 CFR 1910.119(j)(2): The employer did not implement written procedures to maintain the on-going integrity of process equipment: a- Hess Port Reading Refinery - The employer failed to implement written procedures to maintain safety critical instruments to include but not limited to flow, temperature and pressure indicators/alarms/control valves. Such equipment can be found on pressure vessels to include but not limited to T-501R Main Column, D-507R Main Column Overhead Receiver, T-601R Primary Absorber and D-601R Compressor Suction Drum. Condition noted on or about 03/09/09. b- Hess Port Reading Refinery - The employer failed to implement their CUI program with pipe systems identified as susceptible to CUI to include but not limited to piping throughout FCCU, Gas Con and Alky observed with damaged and/or missing insulation. Conditions noted on or about 02/27/09.s
