29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment: On or about March 17, 2015, unused openings in boxes exposed employees to electrical shock and burn hazards in the following instances; (a) In the EPTD Press Area of Building #1, the 220V Control Box to the Metal Reel Loader (Derailer) at press #58 had three unused openings exposing employees to electrical shock and fire hazards. (b) In the Pre-Treatment Area of Building #2, the 480V switch box had an open hole on the right side of the box. (c) In the Stampcoat Paint Room Area of Building #2, the 600V switch box had an open hole on the bottom side of the box.
