29 CFR 1910.146(e)(1): Before entry is authorized, the employer did not document the completion of measures required by paragraph (d)(3) of this section by preparing an entry permit. a) Production Area- Powder Tank; On or about August 22, 2023, and at times prior, employees were exposed to chemical, entrapment and struck-by hazards, in that the employer did not ensure that an entry permit was completed when employees were required to enter the powder tank to perform weekly cleaning duties and maintenance. b) Production Area- Cleaner Tank; On or about August 22, 2023, and at times prior, employees were exposed to chemical, entrapment and struck-by hazards, in that the employer did not ensure that an entry permit was completed when employees were required to enter the cleaner tank to perform weekly cleaning duties and maintenance.
