29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: a) Control panel, Amadas Open Mouth Bagger: On or about November 7, 2017, and at times prior to, employees operating the Open Mouth Bagger were exposed to electric shock hazards where an unused opening in the control panel was not filled. b) On or about November 7, 2017, and at times prior to, employees at the Palletizer were exposed to electrical shock hazards when the cover for the AC Panel L1 was not installed. Live copper wiring was exposed. c) Diesel Switcher outside: On or about November 21, 2017, and at times prior to, employees working in the area near the Coloring Machine and while refueling from the diesel tank were exposed to electric shock hazards when the junction box had an unused opening that was not filled.
