29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: On or about October 08, 2014, and at times prior thereto, the employer did not ensure that the distance between the abrasive wheel periphery(s) and the adjustable tongue at the top did not exceed one fourth inch, exposing employees to the hazards of being struck by flying wheel fragments in the event of wheel breakage. a. In the Machine Shop along the west wall the Jet Equipment & Tools 6 inch bench grinder, adjustment on left side 1/2 inch. b. In the Machine Shop along the east wall the Jet Equipment & Tools 6 inch bench grinder, adjustments left side 1/2 and right side 1 inch. c. In the Maintenance Shop the Jet Equipment & Tools 10 inch bench grinder, left side approximately 1 1/2 inch.
