29 CFR 1910.119(l)(4): A change covered by 29 CFR 1910.119(l) resulted in a change in the process safety information required by 29 CFR 1910.119(d) and the process information was not updated: a) Nustar Asphalt Refining Co. LLC, Paulsboro facility - The employer did not document and update the process safety information when there was a change in alarm set points on PI-622 (pressure indicator) was changed from 14.0 to 17.0, in the distributive control system (DCS) for Unit 2. This is a change of process safety information as described in paragraph d of this section (d)(2)(i)(H). On or about 7/29/09. b). Nustar Asphalt Refining Co. LLC, Paulsboro facility - The employer did not document and update the process safety information when there was a change in alarm set points on the 6-BTU-849 high alarm (heat control) was changed from 106.000 to 107,5000 BTU in the distributive control system (DCS) for Unit 2. This is a change of process safety information as described by paragraph d of this section (d)(2)(i)(H). On or about 7/29/09. c). Nustar Asphalt Refining Co. LLC, Paulsboro facility - The employer did not document and update the process safety information when there was a change in alarm set points on LI-106 (level indicator) from 55 to 70 in the distributive control system (DCS) in Unit 2. This is a change of process safety information as described in paragraph (d)(2)(i)(H) of this section. On or about 7/29/09. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
