29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: a. On or about 11/7/2019, at Assura Windows and Doors LLC, 1543 N Powerline Rd, Pompano Beach, FL 33069. Line #2, A fan (Model No. and/or Serial No. not available) lacked a ground prong on its attachment plug, exposing an employee to an electrical hazard. b. On or about 11/7/2019, at Assura Windows and Doors LLC, 1543 N Powerline Rd, Pompano Beach, FL 33069. Line #2, a two wire extension cord was used to energize a fan, resulting in ground path not being continuous, exposing an employee to an electrical hazard. c. On or about 11/7/2019, at Assura Windows and Doors LLC, 1543 N Powerline Rd, Pompano Beach, FL 33069.
