29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: On or about March 17, 2015, in the Paint Line-STCII and Fabrication Press Areas of Building #2, listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling of equipment and exposed employees to electrical shock hazards; (a) A job-made electrical handy-box with two duplex receptacles (15A, 125VAC/2-pole 3-wire) was plugged (daisy-chained) into a 9-foot extension cord that plugged into the wall. (b) A water fountain installed below the two duplex receptacles (15A, 125VAC/2-pole 3-wire) that was not GFCI protected.
