29 CFR 1910.146(c)(1): The employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces: a) Production Area- Powder Tank; On or about August 22, 2023, and at times prior, employees were exposed to chemical, entrapment and struck-by hazards, in that employees were required to enter the powder tank to perform cleaning and maintenance without the employer performing an evaluation to determine if any hazards were present. b) Production Area- Cleaner Tank; On or about August 22, 2023, and at times prior, employees were exposed to chemical and entrapment hazards, in that employees were required to enter the powder tank to perform cleaning and maintenance without the employer performing an evaluation to determine if any hazards were present.
