Site- The Mitsubishi FG30N, Serial No. AF13F40233, forklift was placed into service with multiple deficiencies including, but not limited to, worn hydraulic hoses, leaking hydraulic fluid, worn tires, and damaged lights b. Site- The Hyster S55XM, Serial No. D187V01582T, forklift was placed into service with damaged tires.
