Text For Citation: 01 Item/Group: 018A Hazard:
29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling: a. On or about February 28, 2018, located in the Maintenance Shop, there was a Dayton Bench Grinder, model number 1Z853, that was plugged into a metal receptacle outlet box with knockouts that was not mounted and not approved to be used as a pendant, thereby exposing the employees to an electrical shock hazard of 120 vac. b. On or about February 28, 2018, located in the Maintenance Shop, the employer mounted a Westinghouse 20 inch box fan to a wall to use as an exhaust fan from the boiler room that was not listed an labeled to be used in an industrial facility, thereby exposing employees to an electrical shock and fire hazard. c. On or about April 19, 2018, located in the Vulcanizing Department, employees used a Westinghouse 20 inch box fan that was not listed and labeled for industrial use, thereby exposing employees to an electrical shock and fire hazard.
