Instance A deleted during Informal Conference B.) Falls Creek Powdered Metals, Inc., 82 Industrial Drive, Brockway, PA 15824, air compressor room, on or about November 23, 2015 and times prior thereto: A flexible electrical extension cord was used to power a fan in the air compressor room. The flexible electrical extension cord was used as a substitute for the fixed wiring of a structure. C.) Falls Creek Powdered Metals, Inc., 82 Industrial Drive, Brockway, PA 15824, production area, on or about November 23, 2015 and times prior thereto: A flexible cord for a lighting fixture on press M4 was plugged in to a receptacle on the opposite side of the wall, and ran through a wall opening to the press. The flexible cord was routed along building surfaces, through wall opening, exposed to damage/chaffing from the sharp metal edge of the metal wall construction.
