LLC, Paulsboro facility - In 2008, a change was made to procedure UN1-03-003, E-401 Overhead Fan Failure. Observed on or about 6/18/09. b) Nustar Asphalt Refining Co. LLC, Paulsboro facility - In 2008, step(s) were added to, and a numerical value (temperature) changed in procedure UN1-04-001, Normal Shutdown Heavy Sour Crude. Observed on or about 6/18/09. c) Nustar Asphalt Refining Co. LLC, Paulsboro facility - In 2008, a change was made to a procedural step and additional step(s) were added to procedure UN1-04-003, H-401 Draining and Panning. Observed on or about 6/18/09. d) Nustar Asphalt Refining Co. LLC, Paulsboro facility - In 2008, changes were made to procedure UN1-05-006, Car Seal Removal/Replacement. Observed on or about 6/18/09. e) Nustar Asphalt Refining Co.
