29 CFR 1910.119(f)(4): The employer did not implement safe work practices for employees and contractor employees to provide for the control of hazards during operationis such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel: a- Hess Port Reading Refinery - The employer failed to implement their safe work practice (Procedure 30-13 Contol of Entry into the Refinery) to account for workers on process units. Both Hess and contract employees failed to sign in/out at the refinery main gate, main control room and process units. Conditions noted on or about 11/19/08, 12/15/08, 12/16/08, 01/05/09 and 04/07/09.
