29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): (a) Sawmill #1 West End - On or about October 2, 2019 a metal electrical box containing a toggle switch for VTB did not have a faceplate exposing employees to electrical shock hazard. (b) Sawmill #1 West End - On or about October 2, 2019 a plastic duplex receptacle box on outside wall of file room did not have a faceplate in place exposing employees to electrical shock hazard. (c) Sawmill #1 West End - On or about October 2, 2019 plastic duplex receptacle box on wall outside of electrical room did not have a faceplate in place exposing employees to electrical shock hazard. (d) Sawmill #3 Electrical Room - On or about October 2, 2019 circuit breaker panel was missing 5 panel blanks exposing employees to electrical shock hazards. (e) Sawmill #3 Electrical Room - On or about October 2, 2019 saw start control box had openings on door exposing employees to electrical shock hazards.
