29 CFR 1910.304(a)(2): Grounded conductors were attached to terminals or leads so as to reverse designated polarity: On or about May 8, 2019, the following electrical receptacles conductors were wired with reverse polarity. 1. At the Flintstone horizontal drilling machinery there was an electrical quadruple box attached to a flexible cord that had receptacles with reverse polarity. 2. On the wall behind the Flintstone horizontal drilling machinery there was an electrical duplex box attached to the wall that had a receptacle with reverse polarity.
