29 CFR 1910.218(h)(4): Tongs were not of sufficient length to clear the body of the worker in case of kickback, and shall not have sharp handle ends: On or about 11/05/2014, the hand tool (a 15" porta-bar) used during forging activities was of an insufficient length and did not prevent the operator from placing any part of the body into the machinery.
