29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: a) The Allen Bradley disconnect labeled 480 volt three phase mounted to the wall in the de barker addition had two knockout plugs missing in the bottom of the disconnect at the time of the inspection on 7/17/2014. b) The large junction box in the chipper electrical room was not securely closed allowing saw dust to enter the cabinet.
