29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: The employer does not ensure that unused opening in cabi nets and fittings are effectively closed to prevent fire and electrical exposure hazards to employees. The following equi pment is compromised to allow for a potential of employee contact (electric shoc k hazard) or contact with dust, insects, water or flammable gases and vapors (fi re hazard). On or about 05/05/10 CSHO observed the following openings in electri cal equipment: A.Covers are missing along covers electrical conduit for the ligh ting circuit around the hydrogen compressors. These openings are located directl y below the steel platform on either side of the back stairs. B.A "Square D" res idential panel located in the water pump area under the pipe rack near the cooli ng tower has a 1" x 2" (2.54 cm x 5.08 cm) opening in the dead front. All the sc rews securing the dead front are missing. C.An electrical conduit elbow fitting cover is missing one screw and is held open by an equipment ground. The fitting is located directly under the burner in the #2 Platformer #4 Heater. Pursuant to 29 CFR 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation that it is in compliance with the standard, including describing the steps it is taking to ensure employees are not exposed to this electrical hazard. One method of abatem ent among others is to repair ground wire on 1PLT-1-F7301, Charge flow indicator , serial #FT-7301, so that the conduit cover can be closed. Check all the electr ical cabinets and covers and replace covers as needed.
